Home Visiting

Home visits can offer much needed support and relief in your parenthood journey.

Home visits promote family wellbeing through information, support, community referrals. The list below includes providers who have opted into the Resource Guide, and therefore may not include all practitioners in Gallatin and Park Counties.

Community Health Partners - Livingston

Parents as Teachers is a free program for all perinatal parents. Specialists meet parents in their home monthly and provide individualized, customized support. They screen the child for developmental milestones and discuss and promote strong, secure parent-child attachment. They bring an activity/game for the parent and child, and give an age-appropriate book to the family at every visit. They connect the family to community resources. They lead monthly support groups. They develop a trusting, caring relationship that lasts for years.

The Home Visiting Program provides in-home assessments of postpartum mothers and children, often based on medical provider referrals or agency requests, offering services like bilirubin testing, postpartum education and developmentally appropriate health support. The program also connects families to community resources and services, fostering comprehensive care and well-being.

Healthy Gallatin Home Visiting

All Healthy Gallatin Home Visiting services are free and can be tailored to specific family needs. Services include developmental milestone information and screenings, breastfeeding education and support, education on infant care and nutrition, as well as assistance in accessing community resources. Through one-on-one visits with a nurse or family support worker, you can build a trusted relationship, get prenatal and parenting support and have your questions answered to better prepare yourself for successful parenthood.

Thrive - The Partnership Project

The Partnership Project is a home visiting program that provides a support network to young families in the Gallatin Valley with children 0-5 years of age. The project is a collaboration between Thrive, Gallatin City-County Health Department and high-quality childcare centers. Services are free and plans are individualized to fit the needs of each family.

Family Outreach

Family Outreach provides free developmental screenings and personalized early intervention home-visiting services to eligible children ages 0-36 months under Montana Milestones/Part C of IDEA. If necessary to support the child’s development, financial assistance is available to cover costs of pediatric therapies, vision, audiology and other vital services and developmental equipment. Family Outreach will help cover the costs of copays, deductibles and services costs for uninsured eligible children.
